Alert Monitoring

Overview:

Alert Monitoring is a new way to identify, follow up on, and document changes in residents' condition. Each night, Fynn reviews your community's data and creates alerts for residents who meet the criteria your community has turned on, such as a significant weight change or no recorded bowel movement in three days. Staff can also create alerts by hand for anything else they want to keep an eye on.

User Benefits:

  • Catch Changes Early: Weight changes of 5% in 30 days, 7.5% in 90 days, or 10% in 180 days are flagged automatically, helping teams notify physicians as regulations require.
  • Clear Workflow: Alerts move from New to Open to Monitoring to Closed, so everyone knows what has been reviewed and what still needs action.
  • No Double Charting: Follow-up progress notes added to an alert also appear in the resident's progress notes history.
  • Visible Everywhere: See alerts on the dashboard widget, the community-wide Alert Monitoring page, or a resident's profile.
  • Simpler Weight Entry: Record weights in pounds or kilograms, exactly as they appear on a doctor's paperwork, and Fynn converts them automatically.

Changes in a resident's health often show up gradually across the information teams document every day. Alert Monitoring brings those changes forward and works alongside your team's clinical judgment, helping communities respond sooner, document interventions consistently, and support compliance. Alert Monitoring is rolling out community by community, with setup and training from the Fynn team.
